RWJBarnabas, CareOne announce acute to post-acute care partnership

The state’s largest health system has partnered with New Jersey’s largest family-owned and -operated senior care provider to further develop an existing relationship focused on transition of acute to post-acute care.

Barry Ostrowsky, CEO and president of RWJBarnabas Health, met with CareOne CEO and President Daniel Straus and CareOne Executive Vice President Elizabeth Straus last week and announced the partnership.

“I look forward to continuing the strong relationship RWJBarnabas Health has developed with CareOne, an organization that has built an outstanding reputation under the leadership of Daniel Straus,” Ostrowsky said. “CareOne’s mission of improving the lives of older adults through quality senior care facilities provides a smooth transition for our hospitalized patients. I anticipate a future of collaboration that will enhance the care of the residents in our communities.”

Many of CareOne’s 32 post-acute care assisted living facilities are in the same communities as West Orange-based RWJBarnabas’s 11 acute care medical centers, according to the announcement.
