The newly introduced model centers on a highly specialized subacute care program for orthopedic recovery and has been immediately adopted by 23 Marquis-supported skilled nursing facilities in the mid-Atlantic region.
OrthoWIN caters to patients recovering from a range of orthopedic surgeries and injuries. Ideal for patients with comorbidities and those without the social support to recover at home independently, the program is directed by a Physical Medicine and Rehabilitation specialist with the support of an interdisciplinary care team. PM&R, also known as physiatry, focuses on restoring health and function with an integrated treatment plan including pain management and physical therapy.
Marquis’ Jennifer Hertzog, vice president of marketing and business development, mid-Atlantic, said the new model responds directly to a call from health care systems for this type of skilled nursing specialization.
“The goal is to help hospital partners address the growing volume of joint replacement care and fracture care patients who are unable to transition directly home following a surgery or injury,” she said. “By formalizing a platform with clear quality measures and a standardized approach, our client skilled nursing centers are well equipped to bridge this gap.”
OrthoWIN features a guided PM&R plan, seven-days-per-week therapy services as warranted, coordination with a patient’s surgical team on preferences, and wound care and management. The platform’s “whole body” approach offers customized support to manage comorbidities with physician-led specialty programming such as cardiac and pulmonary care, chronic kidney disease management, memory care and others. OrthoWIN also includes patient and caregiver education, as well as a virtual or in-person home evaluation prior to discharge.
