Hackensack Meridian Health, Cadence partner to provide remote care delivery system to seniors

Hackensack Meridian Health, New Jersey’s largest health network, and Cadence, a remote care delivery system, announced Oct. 21 a strategic partnership to build the infrastructure to deliver proactive senior care. 

Hackensack Meridian Health wants to extend care beyond clinic walls through connected devices and Cadence’s AI-powered platform, helping seniors stay healthier, more independent and connected to their care teams.

Through the partnership, Hackensack Meridian Health patients use connected devices at home to share vital signs such as blood pressure, weight, heart rate, and blood glucose. Cadence’s AI-powered Proactive Care Engine analyzes this data in real time, alerting the clinical team when intervention is needed and giving patients support with medications and lifestyle guidance.

“Patients deserve care that continues between appointments – care that anticipates needs and prevents problems before they escalate,” said Dr. Anuj Mehta, chief clinical officer, Hackensack Meridian Health. “With Cadence, we are building the infrastructure for that kind of care. It means our clinicians can be proactive without working longer hours, and our patients can count on the same standard of care whether they’re in our exam rooms or at home.”

For clinicians, the program functions as an extension of their practice. Cadence’s clinical team handles daily monitoring, follow-through and patient outreach, while intelligent alerts and care gaps reveal cases that require immediate attention. This approach reduces administrative burden, helps prevent burnout and allows Hackensack Meridian Health providers to focus their time on the patients who need them most. The program is already monitoring more than 1,000 patients.

“This new program gives me the ability to remotely monitor patients at home for chronic conditions such as hypertension and heart failure,” said Dr. Courtney Cunningham, internal medicine, Hackensack Meridian Medical Group, primary care, Oakhurst. “This valuable information helps me better care for my patients through early problem detection and more timely medication adjustments to help keep my patients’ care on track.”

Cadence, based in Staunton, Va., says its programs have delivered measurable results for partner health systems nationwide, including:

  • 70% relative increase in patients achieving guideline-recommended blood pressure goals
  • A 230% increase in heart failure patients on all four pillars of guideline-directed medical therapy
  • $1,300 average annual savings per patient
  • 27% lower hospitalization rate at 12 months post-enrollment
