Bergen New Bridge Medical Center has combined with Dimer Health to launch a transitionist partnership program intended to improve patient outcomes, reduce avoidable readmissions, and close care gaps intensified by food insecurity, and transportation and economic challenges.
Dimer Health is a virtual medical practice of licensed clinicians specializing in transitional care medicine and post-discharge recovery. Dimer Health is supported by a proprietary AI-enabled platform that provides real-time clinical insights, symptom monitoring and direct patient engagement tools.
By combining human connection with leading-edge technology, the program introduces a new clinical role, the transitionist, who stays closely connected with patients following discharge.
