Marquis Health Consulting Services unveils comprehensive Chronic Illness Support care model

Holistic, patient-centric program centers on palliative care and spiritual support

Marquis Health Consulting Services introduced a new, comprehensive Chronic Illness Support model this past week. The platform, which has been immediately adopted by 10 Marquis client facilities, integrates palliative care, pastoral support and other services with specialized clinical programming.

“Marquis’ Chronic Illness Support program emphasizes a holistic focus on both physical and spiritual wellness for chronically ill patients — it’s a next-level model of care for (skilled nursing facilities),” Jennifer Hertzog, vice president of marketing and business development, mid-Atlantic, for the Brick-based nursing home consulting company, said.

Marquis’ patient-centric Chronic Illness Support care model is being directed at each facility by leading regional specialists. It incorporates palliative care, which is administered by a trained team of doctors, nurses and specialists and focuses on providing emotional and spiritual support to help relieve symptoms and stress. The goal is to enhance quality of life for patients and their loved ones.

The Marquis-developed program also weaves in disease-specific education and health coaching, multispecialist-led and -coordinated care, full-time respiratory therapy, social services support and connections to community resources and life enrichment activities. It dovetails with a range of specialized programming — such as cardiopulmonary rehab, chronic kidney disease management, dementia care and others — offered at Marquis client facilities.

“For years, our client facilities have provided exceptional care for individuals with chronic disease through their physician-led specialty programs,” noted Marquis’ Tricia Heller, senior director of specialty services. “The Chronic Illness Support model introduces a palliative care-focused overlay to further improve mental health and wellbeing for these patients and their loved ones. Ultimately, it will support improved health outcomes and reduced hospitalizations within this patient population.”

Beyond its client facilities, Marquis is deeply committed to expanding access to palliative care throughout the skilled nursing industry. To that end, Marquis in 2021 provided financial support for the development of Atlantic Health System’s Community Palliative Care Learning Collaborative. Led by a team of Atlantic Health palliative care and integrated care experts, the initiative aims to provide resources to educate and empower skilled nursing healthcare professionals.

Marquis Health Consulting Services is known for its highly skilled and vision-driven approach. The company currently supports mid-Atlantic and New England facilities with approximately 6,000 skilled nursing and assisted living beds.
