Valley Health System, Inbound Health launch Hospital at Home program

Valley Health System, in partnership with Inbound Health, which helps provide and scale advanced care at home, announced a Hospital at Home program, set to launch this summer. 

This program is intended to deliver high-acuity inpatient care to eligible patients in the comfort and safety of their homes – improving patient experience while maintaining the same standards of quality found in hospitals.

As the United States population ages and the burden of chronic conditions rises, demand for flexible, home-based models has increased. These programs are part of a nationwide movement to not only improve the patient experience but to reduce hospital overcrowding and bed shortages, issues that will become more acute as Baby Boomers age. 

Patients in general like these programs, however, they must meet certain criteria. Generally, they may have to live within 30 minutes of the hospital, in case of emergencies, and they have to have running water, heat, air conditioning, and electricity in their home.

Eligible patients must visit the Valley Hospital, where their care team will assess their medical condition and home-safety protocols. If deemed eligible for the program, the patient’s care team will make a referral to Valley’s Hospital at Home program.

Patients in the Hospital at Home program will receive daily clinical assessments; 24/7 monitoring; medical supplies; and home visits from nurses and clinicians for inpatient diagnoses, such as pneumonia, congestive heart failure, or cellulitis. The overall goal of the program is to help patients heal in a comfortable, familiar environment.

Inbound Health helps enable care teams to extend high-acuity care into the home. According to Inbound, as a leader in advanced care-at-home enablement, the organization brings expertise in scaling these models in alignment with clinical, operational, and financial goals.

The partnership between Valley Health System and Minneapolis-based Inbound Health leverages Inbound Health’s virtual and in-market care teams, technology, logistics, and clinical protocols.
